At Hospice of New York (HONY) our mission is "To Comfort Always". Our diverse team of Provider Relations, Administrative, Management, IT Staff and clinical staff of Registered Nurses, Social Workers, Physicians, Pastoral Care Coordinators, Music Therapists and Home Health Aides have been providing comfort for almost 20 years.

  • Community Hospice RN - Bronx

    Seeking a Full-Time Community Hospice RN

    Responsibilities:
    Provides nursing assessment, planning and care to maximize the comfort and health of patients and families in accordance with the interdisciplinary Plan of Care. Supervises Home Health Aides. Will provide end-of-life care in SNFs, IPUs and homes.

    Qualifications:
    • Registered Nurse licensed in NY state.
    • 2+ years of RN experience (med-surg/homecare/ hospice) preferred
    • Recent BSN graduates will be considered for our mentorship program
    • Bilingual English/Spanish required.
    • Computer proficiency.
    • Ability to work with limited supervision.
    • Strong interpersonal skills.
    • Collaborative and team-oriented personality.
    • Reliable motor vehicle required.

  • IPU Hospice RN - Bronx

    Seeking a Full-Time Hospice RN

    Responsibilities:
    Provides nursing assessment, planning and care to maximize the comfort and health of hospice patients and families in accordance with the interdisciplinary Plan of Care. Supervise Home Health Aides and LPNs. End-of-Life-Care will be provided in an In Patient Unit . Incumbent should be flexible to visit SNF and homes in the Bronx if needed .

    Qualifications:
    • Registered Nurse licensed in NY state.
    • 1 year of hospice or palliative care experience required.
    • Bilingual English/Spanish preferred.
    • Computer fluency required, experience with EMR a plus.
    • Ability to work with limited supervision.
    • Strong interpersonal skills.
    • Reliable motor vehicle
    • Collaborative and team-oriented personality.

  • Hospice Liaison - Queens and Nassau

    Seeking dynamic RN or Social Worker with exceptional interpersonal skills and a passion for excellent care to join our Provider Relations Team to cover Nassau County & Queens.

    Responsibilities:
    Act as a patient advocate; promoting effective transitions between facilities and hospice services. Act as a primary liaison between physicians, hospitals and other healthcare providers. Provide clinical expertise, medical communication and patient/family education for the successful referral of patients.

    Qualifications:
    • Current NYS RN license or LMSW or MSW
    • 3+ years of RN or SW experience;
    • Exceptional interpersonal and communication skills
    • Ability to work with a diverse interdisciplinary team.
    • Proficiency in Microsoft Word applications,EMR (electronic medical records)
    • Hospice or Homecare experience a definite plus
    • Marketing or Sales a definite plus
    • Reliable motor vehicle • Bilingual, English/Spanish a plus
