  • Bilingual Community Hospice Nurse-Bronx (Full-Time)

    Main Responsibilities: Provides nursing assessment, planning and care to maximize the comfort and health of patients and families in accordance with the interdisciplinary Plan of Care. Supervises Home Health Aides. Will provide end-of-life care in the Bronx community, IPU and SNF.

    Requirements:
    • Registered Nurse licensed in NY state
    • 2+ years of RN (hospice, med-surge or homecare experience) preferred. Will also consider a new RN graduate who will participate in HONY mentorship program.
    • Ability to work with limited supervision.
    • Strong interpersonal skills.
    • Collaborative and team-oriented personality.
    • Bilingual English/Spanish speaking.
    • Must have a motor vehicle to reach patients.
    • Experience with Electronic Medical Records a plus

    • Licensed Practical Nurse - Bronx and Brooklyn

      Job Description: Seeking a Full-Time Continuous Care LPN to provide continuous end-of-life care to hospice patients in their homes. The LPN will also be asked to perform the duties of an On-Call Runner.
      Service areas are The Bronx, Brooklyn, Queens and Nassau County.

      Responsibilities:
      Under the supervision of the On-Call Manager will provide continuous care services, routine patient visits, in-home respite care and personal care for patients.
      As a Runner the LPN will perform emergency assessment visits as directed by the On-Call Triage RN. Coordinates the patient plan of care with the interdisciplinary team.
      Work schedule will be overnight and start time can be from:
      • 5:00PM to 8:00AM – Continuous Care Shift
      • 7:00PM to 8:30AM – Runner Shift

      Requirements:
      • LPN licensed in NY state
      • 2+ years of LPN experience
      • Reliable motor vehicle.
      • Ability to work with limited supervision.
      • Strong interpersonal skills.
      • Collaborative and team-oriented personality. • Reliable motor vehicle required in order to reach patients on our program

      Salary: $25 per hour

    • Home Health Aides

      Seeking experienced HHAs to provide services to hospice patients in SNF, IPUs and private residences in the Bronx, Brooklyn, Queens, or Nassau.

      Responsibilities:
      • Provides personal care including bathing, dressing, grooming.
      • Assists patients to ambulate.
      • Assists patients with elimination up to use of external colostomy.
      • Performs other simple procedures as an extension of therapeutic services in accordance with the hospice plan of care.
      • Homecare or Hospice experience preferred

      Requirements:
      • HHA Certificate.
      • 2 + years of HHA experience;
      • Exceptional interpersonal skills
      • Reliable motor vehicle for aides in Nassau
      • Ability to work with a diverse patient population.

    • Provider Relations Assistant - Nassau County

      Main Responsibilities: Supports the Provider Relations Department in the effective completion of the referral process. The Provoder Relations Assistant will work closely with the community Hospice Liason and the facility Hospice Liason. Duties include but are not limited to; obtaining insurance verifications, communicating daily with the referral sources and providers, organizing and distributing of referral packages. Will also be collaborating closely with other departments throughout the organization.

      Requirements:
      • Bachelor's degree in Social Work or Pyschology.
      • 1-2 years experience as an administrative assistant
      • Proficiency in Microsoft Office applications
      • Demonstrates excellent verbal and written skills.
      • Demonstrates organizational skills and the ability to muli-task
      • High degree of professionalism, tact, and sensitivity to confidential information essential
      • Prior experience in a healthcare setting a plus
      • Bilingual English-Spanish or Russian or Mandarin or Cantonese a plus
      • Reliable motor vehicle required.

      Note: This is a Full-Time non-exempt position. The successful incumbant will be eligible for medical, dental, life insurance, LTD, PLT, 401k match and reimbursement for work-related travel expenitures.

      Compensation: $42,000 - $45,000 per year.

    • RN Staff Educator

      Seeking a strong hospice RN and advocate for end-of-life care, with excellent communications skills and a desire to share knowledge and exeperiences with other healthcare professionals.


      Main Responsibilities: Develops, coordinates, and presents training and development programs for all employees. Responsible in identifying company-wide development needs and tailors training specific to learner needs. Also responsible for planning and coordinating training and development activities for our contracted facilities and contracted vendor staff.

      Position is located in Long Island City, Queens, NY

      Requirements:
      • BSN required, MSN preferred.
      • NYS RN License required
      • 3+ years in hospice or palliative care nursing
      • 2+ years in Training and Development, or leadership experience
      • Exceptional written and oral communication skills
      • Proficiency in Microsoft Office applications
      • CHPN certification required within 6 months of hire
      • Exceptional interpersonal skills
      • Must have a reliable automobile, valid driver's license, and the minimum state required liability auto insurance.

      Note: This is a Full-Time exempt position. The successful incumbant will be eligible for medical, dental, life insurance, LTD, PLT, 401k match and reimbursement for work-related travel expenitures.

      Compensation: Commensurate with credentialing and experience.
