About this policy
Hospice of New York ("HoNY") publishes this app for iPhone and Android. The app collects information only when a user sends a referral, and we use it only to respond to that referral. We never sell it.

What we collect
When a user sends a referral, the app sends HoNY what the user enters: the user's name, role and contact details; the patient's name, date of birth, location, diagnosis, care needs and notes; and any photos or PDFs the user attaches. Photos taken in the app are not saved to the user's photo library.
The app has no accounts and uses no advertising or tracking. It does not collect the user's location or read the user's contacts. It stores the user's language choice on the phone only.
How we use and share it
The referral goes by secure email to our admissions team, who use it to contact the user and arrange care. Microsoft hosts the service that delivers it, under agreements that require them to protect it. HoNY shares information with the patient's care providers only as HIPAA permits, or when the law requires.
How we keep and protect it
Referrals are encrypted in transit, and only staff who handle referrals can read them. The app deletes its copies once a referral is sent. If the patient is appropriate, the referral becomes part of their medical record.
User choices
The form is optional. Users can call HoNY instead, 24 hours a day: Metro NY 718-472-1999, Nassau County 516-222-1211. To see, correct or delete a referral, a user can contact HoNY using the details below.
Children and changes
The app is meant for adults. If HONY changes this policy, HoNY will post the new version here with a new effective date.
